Zapier recipe
Send QR Form Responses to Google Sheets
Forms, surveys, polls and feedback pages behind your QR codes already collect responses in your dashboard. This Zap also adds each one to a Google Sheet as it arrives, so your team can work from the spreadsheet they already use.
When this happens
ScanOrbit: New Form Response
Do this
Google Sheets: Create Spreadsheet Row
You need: ScanOrbit Business (the Zapier app is included), a Zapier account, and the ScanOrbit app added to Zapier from our invite link.
Set it up
- Step 1
Make the sheet
One column per field you collect, plus Page name, Rating and Submitted at.
- Step 2
Trigger: ScanOrbit → New Form Response
Fires when someone submits a form, survey, poll or feedback page behind one of your codes.
- Step 3
Action: Google Sheets → Create Spreadsheet Row
Map each response field to its column.
- Step 4
Optional: a Filter step
Only continue for one page name, or for ratings of 3 or below to alert your team.
Fields
| Page name | Which form or survey |
| Type | form, survey, poll or feedback |
| Rating (feedback) | 1–5 stars |
