Survey QR Code
Multiple questions, one dashboard.
A survey QR code opens a short, branded questionnaire — mix single choice, multiple choice, yes/no, star rating, NPS, and free text in one flow. Answers land on the same Results dashboard as every other response type: charted automatically where a chart makes sense, searchable and exportable where it doesn't.

Static vs. Dynamic Survey QR Code
Understand the core operational differences before rolling out physical prints.
Static Survey QR Code
Permanent Raw Data- Works completely offline with no internet server required.
- ✕Destination cannot be changed after printing — requires full reprint.
- ✕Zero scan counts, geographic telemetry, or device insights.
ScanOrbit Dynamic Survey QR Code
Recommended- Update destination URLs or payloads anytime without reprinting.
- Full real-time scan analytics: city, device, OS, time of day.
- Pick your own short slug, and no ads on the way through.
Why use a Survey QR Code?
Nine question types in one flow: choice, multi-choice, yes/no, star, NPS, text
Automatic charting per question — bars for choice, a score for NPS
Fully branded with your logo, accent colour, and background
Dynamic: add, remove, or reorder questions without reprinting
How to create one — step by step
Build the question list
Add each question and pick its type — a mix of choice, rating, NPS, and text is normal, not an edge case.
Order them with intent
Reorder so easy questions come first; a long-text question at the very end keeps drop-off lower than opening with one.
Brand and place
Add your logo and accent colour, then print on a table card, receipt, badge, or packaging insert.
Read the dashboard, not a spreadsheet
The Results tab charts choice and rating questions automatically and scores NPS on its own — export to CSV only when you actually need the raw rows.

What is a Survey QR Code?
A survey QR code is what a poll or a feedback star rating grows into the moment one question genuinely isn't enough. It's a short questionnaire, built question by question from single choice, multiple choice, yes/no, a 1–5 star rating, a 0–10 NPS scale, and short or long text — mixed in whatever order the questionnaire actually needs. Every answer lands in the same place: a Results tab that charts the questions a chart suits (choice bars, star distributions, an NPS score with its promoter/passive/detractor split) and lists the rest as searchable, exportable text.
The line between a poll, a feedback rating, and a survey is genuinely just length. A poll is one question voted on in passing; feedback is a fixed star-plus-comment for an in-the-moment reaction; a survey is for the case where the real answer needs two or three questions to actually mean something — satisfaction plus why, or a rating plus what to fix. Building all three on one underlying engine means the moment a feedback star rating isn't enough, a survey collects the follow-up with the exact same Results dashboard, not a different tool with a different export format to reconcile.
Question order matters more in a survey than in almost any other QR type, because a survey is the one place someone can quit halfway through. Put the shortest, easiest questions first — a star rating or a yes/no takes no thought — and save free text for the end, once someone has already answered two or three things and is more likely to finish the last one too.
“A survey is a poll or a feedback rating that outgrew one question. The moment 'satisfaction' needs a 'why' next to it, that's the line.”

Best practices for survey qr codes
Order questions from easiest to hardest
A star rating or a yes/no takes no thought; a long-text question asks someone to actually compose an answer. Put the easy ones first so people are already partway through, and invested, by the time the harder question shows up.
Use NPS when you want one trackable number over time
A 0–10 'how likely are you to recommend us' question gets scored automatically into promoters, passives, and detractors with a single NPS score — the one number in this list actually built to compare month over month, rather than read fresh each time.
Don't ask what you can already see in analytics
If scan location, device, or time of day already answers a question, a survey question asking the same thing just adds friction. Reserve the questions for what only the respondent can tell you.
Start a poll or feedback rating, upgrade to a survey later
It's fine to launch with one question and add more once you know what else is worth asking — the same QR code and the same Results dashboard carry over, nothing about switching types loses the history already collected.

4 Golden Rules for 100% Scan Reliability
Avoid the most common print mistakes that cause smartphone camera recognition failures.
Always maintain a clear, unprinted margin (the "quiet zone") around the perimeter of the QR code equal to at least 4 modules in width. Without this white border, camera optical sensors cannot distinguish the code from background artwork.
The foreground modules must be significantly darker than the background. Light colored QR codes on white paper or dark codes on reflective foil surfaces fail in low-light environments.
When embedding a brand logo in the center of the QR code, ScanOrbit uses Reed-Solomon Level H error correction (30% redundancy), ensuring the code scans even with significant visual obstruction.
Never upscale a low-resolution JPG or PNG for billboard or packaging printing. Export infinite-resolution Vector SVG directly from ScanOrbit to prevent blurred pixel edges at 300+ DPI.
