What is a Survey QR Code?

A survey QR code is what a poll or a feedback star rating grows into the moment one question genuinely isn't enough. It's a short questionnaire, built question by question from single choice, multiple choice, yes/no, a 1–5 star rating, a 0–10 NPS scale, and short or long text — mixed in whatever order the questionnaire actually needs. Every answer lands in the same place: a Results tab that charts the questions a chart suits (choice bars, star distributions, an NPS score with its promoter/passive/detractor split) and lists the rest as searchable, exportable text.

The line between a poll, a feedback rating, and a survey is genuinely just length. A poll is one question voted on in passing; feedback is a fixed star-plus-comment for an in-the-moment reaction; a survey is for the case where the real answer needs two or three questions to actually mean something — satisfaction plus why, or a rating plus what to fix. Order questions from easiest to hardest

A star rating or a yes/no takes no thought; a long-text question asks someone to actually compose an answer. Put the easy ones first so people are already partway through, and invested, by the time the harder question shows up.

Use NPS when you want one trackable number over time

Don't ask what you can already see in analytics

Maintain 4-Module Quiet Zone

Always maintain a clear, unprinted margin (the "quiet zone") around the perimeter of the QR code equal to at least 4 modules in width. Without this white border, camera optical sensors cannot distinguish the code from background artwork.

Ensure 4:1 Color Contrast

The foreground modules must be significantly darker than the background. Light colored QR codes on white paper or dark codes on reflective foil surfaces fail in low-light environments.
